I will do my best to communicate with you on the best option to achieve your desired look. Sometimes, unfortunately, the desired look you want may not be the healthiest option for your lashes. Recommendations are at my sole discretion. Due to the personal nature of lash extensions, I cannot guarantee that they will last any specific amount of time. There are many factors, things I have no control over, that can determine this.
Here are a few:
1. Your skin type.
2. Your exercise regimen.
3. Air humidity.
4. Porousness of your lashes.
5. The type of facial wash/moisturizer/skin products you use.
7. How often you cry or your eyes water.
8. Your climate.
9. Chlorine &/or saltwater.
That is the short list.
If you are dissatisfied with your new full set, you have options.
I will remove them at NO CHARGE or I will give you a complimentary LASH FIX appointment within 3 days (72 hours) of your original service.
Absolutely no guarantees on fills done by me over other lash artist work.
Please call me immediately so i have time to fit you in for this service. There are NO REFUNDS on services rendered. None. Nada. Please don't ask. Please make sure you express thoroughly what your expectations are for your new, beautiful full set of extensions.
· I require a 24-hour cancellation notice. No show/no call customers will be charged 100% of the service total. Any deposit will be forfeited & an invoice will be sent for the remainder of your balance. This will need to be paid before the booking of another appointment.
If you call or email within the 24-hour period, thereby giving me some sort of notice, you will be charged only $25 or 25% of your service total. You will be sent an invoice to pay this before you may book another appointment with me.
· If you have a package with me, no shows will result in the forfeiture of one service off your package. For late cancelations you can choose between the $25/25% charge or the forfeiture of a fill on your current package.
· Please note that any fees paid for SERVICES RENDERED are non-refundable. Non-Refundable. Please see above for my guarantee to you.
· All prepaid services - packages, BOGOS, etc - have a 6 MONTH expiration date. Any exceptions to this will be clearly noted IN WRITING to you.
· Any fees for services paid through 3rd party vendors (GROUPON, LivingSocial, Gift Certificates acquired through charity events, etc.) are NOT refundable by me. Please call the customer service number provided on your coupon/voucher/gift certificate or contact the place you received it. No shows on these services will result in the loss of promotional value. Actual value will be issued as a spa credit and will expire 12 months from original appointment date.
· Due to the personal nature of cosmetics, lotions, & jewelry, any product purchased from me may be returned within 30 days for a full refund if unopened and in original packaging.
In consideration of other clients, please leave your young child at home. No children under the age of 8 will be permitted to wait for you while you are in your appointment. You WILL be asked to reschedule your appointment.
Please SILENCE your cell phone when entering. As cute as your ringtone is, nobody else wants to hear it. That also goes for the conversation with your sister. This is a shared space. We want to be considerate of others.
And finally, please refrain from smoking outside our front door. We are a place of health & beauty. Smoking is NOT healthy & not beautiful. We care about you and your health, but more importantly our non-smoking clients would greatly appreciate fresh air as they are walking into our shop. Besides, lighting up is hazardous to your lashes!

If you choose for us not to reveal your identity, we will do the best we can to only showcase the area in which the work was done. For example: instead of using your entire face in the photo of a new set of extensions, we would crop the photo so only the eyes are showing & refrain from tagging you and/or using your name on social media.
______Eyelashes - You hereby agree to release the owners, operators and manufacturers from any damages that you might incur due to the application of any products or services performed. You are stating that you understand that the adhesive used for the eyelash extensions is high fume and can be extremely irritating to the eyes. You also understand that some, but very few, people have an allergic reaction to the adhesive causing temporary puffiness to the eyelid. In this circumstance, it is highly recommended that you come back and have the lashes removed or see a doctor. Our glue is LATEX & FORMALDEHYDE FREE! Semi-permanent eyelash extensions wear differently on everybody. Although we hope the initial set will last 3-4 weeks before needing a fill, we cannot guarantee this. In some rare cases your lashes may fall off in the first week. Please make sure you are clear on the care instructions before you leave.
In rare cases any one of the following may occur:
Allergic reaction to the glue. (Cyanoacrylate & Carbon Black are the main ingredients.)
Eye irritation & redness. (Usually lasts only a couple of hours.)
Loss of natural lashes.
Disturbance &/or disruption of vision.
Premature shedding of natural lashes.
Please feel free to ask us about any of these very rare side effects.
There are NO REFUNDS on eyelash extensions. There are also NO REFUNDS on lash extension pre-purchases/packages. Please be sure you LOVE your lashes before purchasing ahead.
______Tanning - I understand that overexposure to DHA does not cause skin damage but can result in possible ORANGE distortion of the skin. Please take your providers recommendations on color level in to consideration. When you use the spray tan process you do not experience protection from the natural sun. Use sun-screen if you plan on going out in the sun. Always avoid overexposure to the natural sun. If you are receiving a spray tan in your home, in a friend’s home, at Shimmer studio, or any other location, you are hereby releasing Shimmer of ANY damage or injury liability and/or any costs associated. Tanning solution can stain carpets, furniture & clothing, especially if spilled. Be sure to protect your property. You agree by signing this form that you have no medical condition, allergies, or other reason that would prohibit you from sunless tanning. You have been given adequate instructions for the proper use of the sunless application via instructions on the Shimmer Skin Lash & Tan web site, you understand the risks involved. You understand that everyone tans differently and certain medications, health conditions, and monthly cycles can affect your results. You are tanning at your own risk. You hereby agree to release the owners, operators and manufacturers from any damages that might occur due to the application of this spray tan. There are NO REFUNDS on airbrush tans. There are also NO REFUNDS on tan packages.
______Waxing/Sugaring - Bruises, scabs, scarring, redness, hyper pigmentation or pimples can occur with hair removal. Severe skin lifting can occur when taking certain medications. Although extremely rare, waxing/sugaring of soft tissue may cause the skin to tear resulting in the need for stitches. (Most common occurrence is in Brazilian Bikini waxes, male or female.) You agree to advise us of any allergies to latex as we sometimes use latex gloves for the waxing procedure. You understand all of the above-mentioned reactions. You also understand if you change your skin care routine or medications you must inform the professional PRIOR to starting any service in the future. We prefer that you do not tweeze your hair. This takes some getting used to on your part, but the reason for this is that it will make your waxing service results last longer. Perfect waxing means wax is applied and removed and all the hair comes off. Now you have a smooth, stubble free area that remains hairless in-between appointments. This only occurs if the hair is long enough to be pulled out by the root. Tweezing interrupts this from happening. So, we ask that you be patient for about 1 month, don’t tweeze, shave or use a depilatory cream, come in every 3 to 4 weeks like clockwork and you will love your waxing even more than you do now!
